About Social Worker the role:
-Conduct comprehensive assessments of individuals and families to identify social, emotional, and economic needs.
-Provide counseling and support to address issues such as mental health, family dynamics, and life transitions.
-Advocate for clients’ rights and access to resources within the community.
-Collaborate with other professionals to ensure clients receive comprehensive care.
-Respond to crisis situations and provide immediate support and intervention as needed.
-Develop crisis intervention plans for individuals and families in distress.
-Develop and implement individualized care plans to address clients’ specific needs.
-Coordinate with other service providers to ensure a cohesive and effective support network.
-Connect clients with appropriate community resources, such as housing assistance, financial aid, healthcare services, and support groups.
-Conduct educational programs and workshops to promote mental health, wellness, and prevention of social issues.
-Provide information on available resources and support networks.
-Maintain accurate and up-to-date client records, including assessments, progress notes, and service plans.
-Ensure compliance with confidentiality and legal requirements.
-Work collaboratively with interdisciplinary teams, including healthcare professionals, educators, and community organizations.

Job Requirements
-Bachelor’s Degree in Social Work/Licensed Social Worker (LSW) in Ohio
-At least one year of experience in the field working directly with DD/IID individuals
preferably in a residential setting
-State licensure as a Licensed Clinical Social Worker (LCSW) or equivalent.
-Previous experience in social work, counseling, or related fields.
-Knowledge of community resources and social service agencies.
-Strong interpersonal and communication skills.
-Empathy, cultural competence, and the ability to build rapport with diverse populations.
-Ability to work independently and as part of a collaborative team.
